Nursing & Clinical Care (RN-Led)
Post-hospital discharge monitoring, complex wound and wound vac care, chronic disease oversight, palliative care, medication management — all BCCNM-compliant and fully insured.

Call (778) 837-6727 or email referral@nashobahomecare.ca. A nurse speaks with you directly. Discharge planners can refer patients — we respond the same day.

In-Home Nursing Assessment
A BC-licensed RN visits the patient at home. We assess wounds, medications, vitals, cognitive baseline, and full clinical need — establishing a documented starting point.

BCCNM-Aligned Care Plan
A precise, BCCNM-aligned clinical care plan is developed — focused on the medical goal: heal the wound, stabilise the condition, support safe recovery at home.

Skilled Nursing Visits Begin
Dressing changes, wound vac management, medication reconciliation, chronic disease monitoring, catheter care — all documented and reported back to your physician.
